LIABILITY WAIVER AND RELEASE FORM
TRISHNA GRIGOWSKI LLC d/b/a YOGA WITH TRISHNA

Effective Date: July 3, 2025

WAIVER AND RELEASE OF LIABILITY

By checking this box, I acknowledge and fully understand that I will be engaging in physical activities that may involve some risk of injury. I am fully aware of the risks and hazards involved.

I hereby affirm that I am in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in these activities.

I fully understand that it is my responsibility to consult with a physician prior to and regarding my participation in yoga classes, workshops, or any other activities with Trishna Grigowski LLC. I acknowledge that I have no physical impairments, injuries, or illnesses that will endanger me or others.

In consideration of being permitted to participate in yoga classes and other activities conducted by Trishna Grigowski LLC, I, for myself, my heirs, personal representatives, or assigns, do hereby release, waive, discharge, and covenant not to sue Trishna Grigowski LLC, its instructors, employees, or agents from liability from any and all claims including negligence resulting in personal injury, accidents, or illnesses (including death), and property loss arising from, but not limited to, participation in the yoga program.

ASSUMPTION OF RISKS

I understand that yoga includes physical movements as well as an opportunity for relaxation, stress re-education, and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will listen to my body, adjust the posture, and ask for support from the instructor.

INDEMNIFICATION AND HOLD HARMLESS

I also agree to indemnify and hold harmless Trishna Grigowski LLC, its instructors, employees, or agents from any and all claims, actions, suits, costs, expenses, damages, and liabilities, including attorney's fees brought as a result of my involvement in the yoga program.

PHOTO/VIDEO RELEASE

I consent to and authorize the use and reproduction by Trishna Grigowski LLC of any and all photographs and video recordings taken of me for promotional purposes. I waive any rights, claims, or interest and I understand that I will not receive any financial compensation.

SEVERABILITY

I further expressly agree that the foregoing provisions in this waiver and release agreement are intended to be as broad and inclusive as permitted by the laws of the state in which Trishna Grigowski LLC operates and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

ACKNOWLEDGMENT OF UNDERSTANDING

I have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am accepting this agreement freely and voluntarily by checking this box, and intend my digital acceptance to be a complete and unconditional release of all liability to the greatest extent allowed by law.
